This plan offers the benefits comparable to a preferred provider dental insurance plan.

There are NO yearly maximum amounts and NO deductibles!

Our Membership Plan includes the following services:

• 2 Exams
• 2 Regular Cleanings
• All Necessary X-Rays
• 1 Fluoride treatment (for all ages)
• 1 Emergency Exam

Members receive a 15% savings on the following:

• Fillings
• Implants
• Sealants
• Root Canals
• Extractions
• Periodontal Treatment
• Crowns
• Bridges
• Veneers
• Nitrous Oxide (Laughing Gas)
• Dentures and Partials
• Occlusal Guards

Items not covered by Membership Savings: Oral healthcare products and procedures covered by other plans.

The membership fee for each adult member is $350.00 for a year.

The membership fee for children 13 and younger is $250.00 for a year.
