This plan offers benefits comparable to a preferred provider dental insurance plan.
There are no yearly maximum amounts and no deductibles with the membership plan.
Our Membership Plan includes the following services:

- 2 Regular Cleanings
- Annual Routine X-rays
- 1 Fluoride Treatment (Adults)
- 2 Fluoride Treatments (Children)
Members receive a 15% savings on the following:
- Fillings
- Implants
- Sealants
- Radiographs
- Root Canals
- Extractions
- Periodontal Treatment
- Crowns
- Bridges
- Veneers
- Nitrous Oxide (Laughing Gas)
- Dentures and Partials
- Occlusal Guards
Items not covered by Membership Savings: Oral healthcare products and procedures covered by other plans.
The membership fee for each adult member is $375.00 ($620+ Value Minimum) for a year.
The membership fee for children 13 and younger is $275.00 ($540+ Value Minimum) for a year.
